An aircraft goes quiet over the ocean. The expected position report does not come. Somewhere on the ground, a clock starts, and a chain of decisions begins that may end with rescue aircraft and ships converging on a patch of empty sea thousands of miles from anywhere. Who organizes that response, who pays for it, and who is allowed to cross a border to help: those questions have to be answered before the emergency, not during it. Annex 12 answers them.
Annex 12 to the Convention on International Civil Aviation governs search and rescue, SAR, for aircraft in distress. Its premise is a moral and practical one: a state that controls a piece of airspace or sea accepts the duty to look for and rescue those who come to grief in it, regardless of their nationality, and to cooperate with its neighbors so that no one is lost simply because they went down on the wrong side of a line.
The map of responsibility
SAR works because the entire globe is divided into areas of responsibility, leaving no gaps. The world is carved into search and rescue regions (SRRs), each assigned to a state that takes responsibility for SAR within it. Coordinating each region is a rescue coordination center (RCC): the operational hub that receives the alert, assesses it, and directs the response, drawing on rescue subcenters and the actual rescue units. The SRRs are deliberately drawn to cover oceans and remote areas that belong to no one, so that even a ditching in the middle of an ocean falls within some state's defined duty to respond.
The three phases of an emergency
Not every missed radio call is a catastrophe, so Annex 12 defines a graduated set of emergency phases that scale the response to the level of concern. They escalate in seriousness:
- The uncertainty phase (INCERFA): there is doubt about an aircraft's safety, perhaps because a report is overdue. Watchfulness begins.
- The alert phase (ALERFA): apprehension about the aircraft's safety has grown, and the response machinery is readied.
- The distress phase (DETRESFA): there is reasonable certainty the aircraft is in grave and imminent danger. The full rescue effort launches.

Cooperation across borders
A defining feature of SAR is that distress does not respect borders, so the response cannot either. Annex 12 obliges states to cooperate, and, crucially, to permit the entry of other states' rescue units into their territory when needed, cutting through the sovereignty and customs formalities that would otherwise cost lives. Over the vast oceanic and remote regions where one state's SRR meets another's, that shared responsibility is the difference between a coordinated search and a fatal jurisdictional standoff. The principle is simple: get the rescuers to the survivors first, sort out the formalities later.
A border is an argument for peacetime. In a distress phase, Annex 12 tells it to step aside.
The tools of a search
Annex 12 covers SAR operating procedures and the resources states must arrange, and it leans on a piece of technology that has transformed the field. The COSPAS-SARSAT satellite system detects and locates distress signals from emergency beacons: on aircraft, the emergency locator transmitter (ELT). When an aircraft crashes, the ELT is designed to activate and transmit a signal that the satellite constellation picks up and relays to the nearest RCC, turning a needle-in-an-ocean search into something far closer to a guided one. The Annex also standardizes the signals and communications used in SAR, so that aircraft, vessels, and survivors can be coordinated and understood across the languages and services involved.
How it connects to the rest
SAR is the receiving end of alerts generated elsewhere in the system. It is triggered by the alerting service defined in Annex 11: the air traffic service that notices an aircraft has gone silent or overdue is the one that wakes the RCC. And it intersects with Annex 6, which places obligations on operators, including, increasingly, the tracking of their aircraft so that a missing one can be located faster. The chain runs from the controller who notices the silence, to the RCC that classifies the phase, to the rescue units that launch.

The disappearance of Malaysia Airlines Flight MH370 in 2014 exposed an uncomfortable truth: in an age of satellites and constant connectivity, a large airliner could vanish and leave the world unable to find it. The search consumed years and vast resources and still did not produce certainty. The response was a concerted push on aircraft tracking, embodied in the Global Aeronautical Distress and Safety System (GADSS): a framework of requirements for tracking aircraft in normal flight and, critically, for autonomous distress tracking that reports an aircraft's position frequently when something goes wrong, so the search area for the next MH370 is measured in a small radius rather than an ocean. It is a textbook case of the system learning from its worst failure.
How states implement it
States provide SAR through their own arrangements, often blending civil and military resources. Saudi Arabia organizes SAR through the General Authority of Civil Aviation (GACA) in coordination with national rescue resources. The United States runs a layered system in which the FAA, the Coast Guard, and, for inland aeronautical SAR, the Air Force Rescue Coordination Center (AFRCC) divide the responsibility. European states maintain their own national SAR arrangements coordinated across the continent. In every case the structure mirrors Annex 12: defined regions, coordinating centers, and cooperation across the lines between them.
What a rescue actually involves
Behind the tidy structure of regions and centers is a messy, high-stakes operation, and Annex 12 does not pretend otherwise. A real search may span sea and land, civil and military assets, aircraft and ships and ground parties, often in bad weather and failing light. Because so many distress cases over water blur the line between aviation and maritime rescue, ICAO and the International Maritime Organization maintain a joint reference, the International Aeronautical and Maritime Search and Rescue (IAMSAR) Manual (Doc 9731), so that an aircraft ditching and a ship in distress are handled within one compatible system rather than two that cannot talk to each other.
One of SAR's quietest resources is other aircraft. A flight near a distress location can be asked to divert, descend, and act as an on-scene relay or visual searcher until dedicated units arrive: a duty that flows from the same shared obligation the Annex is built on. The rescue coordination center orchestrates all of it: classifying the emergency phase, defining the search area from the aircraft's last known position and likely drift, tasking the assets, and adjusting as information comes in. It is improvisation inside a framework, and the framework is exactly what Annex 12 provides, so that the improvisation starts from organization rather than chaos.
Where it still falls short
The hard problems in SAR are problems of distance, speed, and money.
- Coverage over vast oceanic regions. The SRRs leave no gaps on paper, but covering them in practice, reaching a remote ditching quickly enough to matter, strains even well-resourced states.
- Cross-border coordination speed. The duty to cooperate is clear. Doing it fast, across different agencies and procedures in the first crucial hours, is harder than the standard makes it sound.
- Resourcing. SAR assets are expensive and rarely used, which makes them a constant target for underfunding, until the day they are needed and found wanting.
There is something that sets Annex 12 apart from most of the others. Most of aviation regulation is about preventing the bad day. This Annex is about the bad day having already arrived: the aircraft is down, and the only question left is whether anyone reaches the survivors in time. It is the part of the system that exists not to keep flight safe, but to honor the people for whom safety failed. That a state will look for you over an empty ocean, whoever you are, is not a small thing. It is one of the quieter promises aviation makes.
